Desflurane (also known by its English name: Desflurane), or desflurane for short, is a halogenated ether anesthetic primarily used to sustain anesthesia. Similar to other halogenated ether anesthetics, it is administered as a racemic mixture. In modern clinical settings, desflurane and sevoflurane are increasingly supplanting isoflurane as preferred inhalational anesthetics. Owing to its low blood solubility, desflurane demonstrates the most rapid induction and recovery among all volatile anesthetics, with drug effects wearing off swiftly. However, it has certain limitations, including relatively lower anesthetic potency, a sharp, irritating smell, and a high production cost. In regions with limited healthcare budgets, this has hindered widespread adoption. That said, in low-flow anesthesia techniques, the cost gap between desflurane and isoflurane becomes nearly negligible [1]. At vapor concentrations above 10%, desflurane may induce elevated heart rate and provoke airway reactivity, which is why it is rarely used for anesthetic induction.

Anesthesia delivery systems designed for desflurane typically incorporate a dedicated heating unit to maintain a stable temperature, minimizing fluctuations in vapor concentration caused by ambient conditions. It is well-documented that desflurane—along with enflurane and, to a lesser extent, isoflurane—can chemically react with carbon dioxide absorbents commonly used in anesthesia circuits, resulting in decomposition and the production of trace yet measurable levels of carbon monoxide. Conditions such as absorbent desiccation or use in high-insufflation settings can significantly amplify this reaction, particularly when Baralyme is used as the absorbent, leading to increased CO generation.
