Indapamide powder is an organic compound with the chemical formula C₁₆H₁₆ClN₃O₃S. It appears as a white or off-white, odorless, and tasteless powder. This substance is virtually insoluble in water or dilute hydrochloric acid, soluble in ethanol and ethyl acetate, readily soluble in acetone and glacial acetic acid, and slightly soluble in chloroform or diethyl ether. Indapamide is a widely used, over-the-counter antihypertensive medication in China. It is valued for its good efficacy, stable blood pressure-lowering effect, and minimal side effects.

Function
Indapamide powder is effective for treating mild to moderate essential hypertension. When used alone, it produces a significant antihypertensive effect without the need to add other diuretics. It can also be combined with beta-blockers.
Application
Indapamide possesses dual diuretic and calcium antagonistic properties. It acts by inhibiting sodium ion reabsorption in the proximal and distal convoluted tubules, producing a diuretic effect, and by blocking calcium ion influx. It shows high selectivity for vascular smooth muscle, dilates peripheral small vessels, and thereby lowers blood pressure. Its effect on vascular smooth muscle is stronger than its diuretic effect; at sub-diuretic doses, it reduces blood pressure, while higher doses exhibit diuretic action.
Unlike thiazide diuretics, Indapamide does not cause orthostatic hypotension, flushing, or reflex tachycardia. It also has no significant adverse effects on blood counts, lipid metabolism, blood sugar, renal function, heart rate, cardiac output, or the electrocardiogram (ECG). It has no obvious effect on the central or autonomic nervous system. An antihypertensive effect is observed 2-3 hours after oral administration and is maintained for 24 hours. The diuretic effect begins at approximately 3 hours and peaks at 4-6 hours.
Distinct from other diuretics, Indapamide has high lipid solubility. After oral absorption, it concentrates primarily in the liver and kidney plasma, with low levels in the heart, lungs, muscle, and fat. It is mainly metabolized, with only about 5% excreted by the kidneys in its original form.
Indapamide is suitable for mild and moderate primary hypertension and can also be used for congestive heart failure associated with fluid retention. It is applicable to patients with hypertension who also have renal failure, diabetes, or hyperlipidemia, showing a remarkable antihypertensive effect. Its efficacy is enhanced when used with beta-blockers. Because it has a diuretic effect, it may cause hypokalemia, so potassium supplementation may be required concurrently.
