Mechanism of Action
Azithromycin functions similarly to erythromycin by targeting the 50S subunit of bacterial ribosomes to inhibit RNA-dependent protein synthesis, retaining erythromycin’s antibacterial profile. While its activity against Chlamydia mirrors erythromycin, it exhibits significantly stronger efficacy against Gram-negative bacteria, including:
- Influenza bacillus & Neisseria gonorrhoeae: 4× more potent than erythromycin
- Legionella: 2× more active than erythromycin
- Enterobacteriaceae: Marked improvement over erythromycin
Its MIC for most Gram-negative pathogens is below 1μg/ml. It also demonstrates effective action against Toxoplasma and Treponema pallidum. Notably, Staphylococcus aureus and Streptococcus pyogenes may show cross-resistance to erythromycin.
Azithromycin is primarily indicated for infections of the respiratory tract, urogenital tract, skin/soft tissues, and select sexually transmitted diseases caused by susceptible microorganisms.

Azithromycin is effective against the following conditions:
