Introduction to Irinotecan
Irinotecan is a water-soluble, semi-synthetic compound derived from camptothecin. Together with its active metabolite, SN-38, it functions as a potent inhibitor of DNA topoisomerase I. By forming a complex with both the enzyme and DNA, it induces single-strand DNA breaks, thereby blocking DNA replication and inhibiting RNA synthesis, particularly during the S phase of the cell cycle.

Mechanism of Action
Extensive clinical studies have demonstrated that irinotecan hydrochloride exhibits significant inhibitory activity against various cancers, including colon cancer, small cell lung cancer, rectal cancer, and leukemia. It remains effective even in cases resistant to fluorouracil, which has garnered substantial global interest in the compound.
Applications
Irinotecan hydrochloride is a first-line treatment for advanced colorectal cancer and is also employed as a post-surgical adjuvant chemotherapy agent. It shows therapeutic potential in the treatment of lung, breast, and pancreatic cancers, among others.
Potential Side Effects
Delayed Diarrhea: This is the most common adverse effect, affecting 80-90% of patients, with severe cases occurring in about 39%. Symptoms typically begin around the fifth day after administration and last an average of four days. Immediate management with oral loperamide is crucial. The initial dose is 4 mg, followed by 2 mg every two hours for at least 12 hours, continuing until 12 hours after the last watery stool (not to exceed 48 hours). If diarrhea persists, a 7-day course of prophylactic broad-spectrum antibiotics (e.g., quinolones) should be considered.
Severe Neutropenia: This condition occurs in approximately 39.6% of patients. Gastrointestinal reactions like nausea and vomiting are common but generally not severe. Prophylactic use of G-CSF or GM-CSF is not typically recommended, though it may be considered if severe neutropenia develops during the first treatment cycle.
Acetylcholine Syndrome: This can include "early" diarrhea (within 24 hours), sweating, increased salivation, visual disturbances, and tearing. For severe symptoms, a subcutaneous injection of atropine (0.25 mg) may be administered. Prophylactic atropine is not advised for the first cycle but may be used in subsequent cycles if early diarrhea or other severe symptoms occur. Other reported mild effects include mucositis, alopecia, fatigue, and skin toxicity.
Hepatic Effects: In patients without progressive liver metastases, monotherapy can cause transient, mild-to-moderate elevations in serum liver enzymes (aminotransferase, alkaline phosphatase) and bilirubin. Transient mild-to-moderate increases in serum creatinine may also occur. Grade 3 elevations are rare, and no Grade 4 elevations have been observed.
