Clomipramine exerts its antidepressant action primarily by blocking the reuptake of norepinephrine (NA) and serotonin (5-HT) in presynaptic nerve terminals. It also delivers anxiolytic and mild sedative properties. Notably, its inhibition of serotonin (5-HT) reuptake is significantly stronger than that of most other tricyclic antidepressants, while its inhibition of norepinephrine is weaker compared to imipramine and amitriptyline.

The compound exhibits moderate anticholinergic activity and relatively low sedative side effects. It is efficiently absorbed through oral administration, achieving therapeutic plasma levels of 200–500 ng/mL within 1–2 weeks. Following liver metabolism, the active metabolite norclomipramine peaks at concentrations roughly double that of the parent drug.

Plasma protein binding ranges from 96% to 97%, with an elimination half-life of 21 to 31 hours. Roughly 70% of excretion occurs via urine, and 30% via feces.
