What is Clomipramine?
Clomipramine is a well-established, potent tricyclic antidepressant known for its fast-acting and dependable therapeutic effects. It appears as a white to off-white or pale yellow crystalline powder at room temperature, is odorless, and has a pronounced bitter taste. Prolonged exposure to light may cause a yellow discoloration.
This compound is highly soluble in glacial acetic acid and chloroform, freely soluble in water and ethanol, slightly soluble in acetone, and practically insoluble in ether.

Function
Clomipramine exerts its antidepressant action primarily by blocking the reuptake of norepinephrine (NA) and serotonin (5-HT) in presynaptic nerve terminals. It also delivers anxiolytic and mild sedative properties. Notably, its inhibition of serotonin (5-HT) reuptake is significantly stronger than that of most other tricyclic antidepressants, while its inhibition of norepinephrine is weaker compared to imipramine and amitriptyline.
The compound exhibits moderate anticholinergic activity and relatively low sedative side effects. It is efficiently absorbed through oral administration, achieving therapeutic plasma levels of 200–500 ng/mL within 1–2 weeks. Following liver metabolism, the active metabolite norclomipramine peaks at concentrations roughly double that of the parent drug.
Plasma protein binding ranges from 96% to 97%, with an elimination half-life of 21 to 31 hours. Roughly 70% of excretion occurs via urine, and 30% via feces.
Clomipramine is clinically used to address:
- Endogenous, neurotic, reactive, and latent depression
- Depressive symptoms associated with schizophrenia
- Obsessive-compulsive disorder (OCD) and phobic disorders
- Chronic physical pain syndromes
It notably reduces 5-hydroxyindoleacetic acid (5-HIAA) levels in cerebrospinal fluid and platelets—a biological marker linked to its efficacy in treating OCD. The molecule demonstrates affinities for D2 dopaminergic, alpha-adrenergic, and histamine receptors, offering additional pharmacological modulation. Furthermore, its pronounced anticholinergic properties and mild dopamine receptor antagonism may stimulate increases in prolactin and growth hormone secretion.
Drug Interactions
- Alcohol: Concurrent use amplifies central nervous system depression.
- Anticonvulsants: May lower seizure threshold, reducing anticonvulsant efficacy; dose adjustments of antiepileptic agents are necessary.
- Antihistamines or Anticholinergics: Effects are enhanced synergistically; dosage reduction may be required.
- Guanethidine: Clomipramine may diminish antihypertensive action, although high-dose guanethidine (above 150 mg/day) remains effective.
- Estrogens (including oral contraceptives): May increase clomipramine-associated side effects and reduce antidepressant effectiveness.
- Monoamine Oxidase Inhibitors (MAOIs): Risk of hypertensive crisis; discontinue MAOIs at least two weeks prior. Deaths have been documented from such combinations.
- Adrenergic Agonists (e.g., cocaine, epinephrine): Could precipitate severe hyperthermia and hypertension.
- Thyroid Hormone Preparations: May act synergistically, leading to arrhythmias; reduced doses of both agents are recommended.
Application
- Treatment of multiple depressive subtypes
- Management of obsessive-compulsive disorder and phobic neuroses
- Supportive care for pain disorders involving neurochemical imbalances
